Our Team:
Our trained medical professionals and volunteers meet regularly to address the unique needs of each patient and his or her family and work together to create a community of compassionate care centered around the patient.
Our Services:
- Skilled Nurses who are trained specifically in pain and symptom management. Available 24 hours a day
- Social Workers offer counseling for the whole family. In unison with the family/patient dynamics, they act as resources for a variety of community services.
- Home Health Aides and Homemakers certified and provide personal care.
- Physicians work with the patient’s doctor and are available for pain and symptom management consultations.
- Chaplains provide the necessary spiritual support to the patient and the family
- Bereavement support for families and the community.
- Volunteers spend time with patients and provide assistance through visits, letter writing, reading, or shopping.
- Medication and Medical Equipment we provide basic medical equipment, supplies and medications related to hospice diagnosis.
- Crisis Care including in-home pain and symptom management wherever the patient calls home.
Hospice in a Home-like Setting
Silveroak Hospice partners with multiple nursing homes and assisted living facilities.
Hospice considers the nursing home staff to be a part of the patient’s family and, when appropriate, includes the staff in the circle of its care and concern.
Hospice does not replace nursing home care, but rather complements it by providing the same palliative services that our home patients receive.
