Refer a Patient to Hospice
Upon receiving a referral, a member of the Silveroak Hospice Team will contact the patient and family to arrange for an admission consultation. Admission consultations are conducted at the location of the patient and family’s choosing. Consultations are conducted 24 hours a day, 7 days a week. The following is a list of individual that can refer a patient:
- Family Physician
- Family Member
- Medical Director
- Clergy
- Nursing Home or Assisted Living Center Member
- Guardian
- Friends
During the consultation the patient and family will be provided with a complete outline of the hospice program, services offered and have the opportunity to ask all of the questions and concerns they may have.
